FYI, here is Mike's emailed response to my question posted above:

Jon,
Injuries are TURNED ON during the playoffs and he could be lost---it is a
natural progression from the regular season.

Secondly, players can be injured up to their occurrences and do not
necessarily get hurt the exact amount of occurrences or games.
