Sometimes there are legitimate crashes which keep a game from being reported. I have had them in games that I WON AND games that I LOST. So you should monitor the situation closely and ask questions if something looks fishy. If something looks strange keep a written log. If you see the same guy having game 5 not reported often then I would confront him and/or send e-mail to the commish.

But if its an isolated incident it could be a legitimate problem with not being able to report the game.
