I've had the same problems. I,ve always been able to get it set eventually though, sometimes after 3 or 4 tries. I've tried all the things you've tried and eventually it works, it's just time consuming.
When I go out of the lineup setup and then back in, the Pinch-hitting default always moves and has to be reset where I want it.
I think this is a new glitch because I never had this problem in previous years.
