Both these problems - the infuriating lineups and the irriating PH sitting - are common and have been on this board in the past.

To fix your lineups, don't fight it -- you can't win. Quit out, clear your browser cache, and then log back onto PB. It will work. (My "favorite" was when I went to set-up my second team and the lineups from the first kept popping up!)

Re the PH setting, just keep checking it. I don't think anybody has found a fix.
