First you right click the pinch runner to get a list of positions, then simply pick one.

To do a double switch, just highlight the reliever and the player whose lineup slot the pitcher will go into and hit lineup change. Put the pitcher as a pitcher. Then put the replacement fielder in the pitchers slot the same way.
