When you say that the Robot played "All 8 players out of position for the whole game" I am assuming that they started that way (as opposed to coming in as a pinch-hitter and playing out of position).
In home games, here's what triggers that.
You set your lineup and click submit. The system seems to be taking a long time, so you click submit again. Bingo, all players will appear out of position.
I don't know if this has ever happened in an away game, but that's my guess.

The other thing that can happen w/ away games is this.
You set your roles and away lineups and then farm out a player who is in the away lineup. Since the lineup is no longer valid, I THINK that the pitching roles are ignored as well (Not sure about this).

The other thing that I've noted is that if I have 2 relievers assigned to the same role, one will get used often to the exclusion of the other. One of my traditional teams had Rhodes and Quantrill. Quantrill's good, Rhodes is great. Very often, after an away series, Quantrill comes back rusty and Rhodes pitched 4+ innings in one game. I think I can overcome this by who I pair together, but I haven't found the key yet.
